B. Pre-Arrival Information

Please submit the following forms:

1) International Student Arrival Form   DOWNLOAD
2) Housing Preference Form   DOWNLOAD
3) Health Forms: Confidential Health History, Physical Examination, Immunization Record
     and Meningococcal Form

1) International Student Arrival Form   DOWNLOAD

The Office of International Programs provides transportation services from airports to our campus for new international students. Please complete an International Student Arrival Form and return it by email to leeh01@centenarycollege.edu or fax to 1-908-366-7592. The college must receive this form at least two weeks before your arrival.

Note:
- Planning your airport arrival:
Newark International Airport in Newark, New Jersey is most convenient due to its proximity to the College and its efficient customs authority (students have gotten through customs in less than half an hour). It is safer than larger airports such as JFK in New York, as well as more manageable for the first time international traveler. If you are unable to arrive at Newark Airport, JF Kennedy International Airport in New York would be the second choice.

- Date and time of arrival:
We understand this difficulty and will make every effort to assist you upon your arrival by whatever means is necessary. It is very important to notify the College of any changes in your arrival plans, including flight delays. Messages can be received at the switchboard (908-852-1400) at any hour, even during holidays, and your message will be relayed to the appropriate person.

2) Housing Preference Form   DOWNLOAD

Many new resident students are concerned about their future roommate. This Housing Preference Form is a guide that will be used to help Campus Life pair you with a roommate and residence hall that will successfully transition you to a Centenary College resident. Please download this form and take a few moments to respond to this questionnaire as honestly as possible.

3) Health Forms   DOWNLOAD

New Jersey State Law requires that all new students see their doctor, have these forms completed, and returned to Health Services. Please send original copies to the following address:

4) Purchasing Additional Health Insurance  

All Centenary students are charged a group health insurance fee (as part of their semester bill) that entitles them to the benefits of the College’s medical coverage. The purpose of this plan is to provide very basic hospital and surgery coverage for both accident and sickness to all the College’s students.
This plan is very limited and does not cover routine physical examinations, dental expenses, or other types of preventive care. Students who require this form of healthcare will have to pay their doctor or dentist directly for services provided. This plan also does not cover services of a specialist in the event of a condition that requires this type of care. Students who developed a condition requiring on-going medical care after their arrival on campus have occasionally incurred extremely high medical bills.
For this reason, additional insurance to cover these costs is highly recommended. The Health Services Office on campus is available for assistance in case of minor illnesses and injuries, at no charge to students.
